Practical weaknesses of passwords

So why are are passwords schemes so easy to break?

     The effort to remember a good password is much greater than the effort to choose a weak password.

We often pick a password that is easy to remember such the name of a pet or a birthday. Such choices drastically reduce the number of attempts needed to guess the correct password.

 If we choose a password that is more difficult to guess it is naturally harder to remember. We often compromise this password by writing it down where it can be found.

We further compound this compromise by using the same password for multiple applications. Therefore if this single password is discovered, many applications instead of just one, become insecure.

Worse yet, we don't change passwords very often as all this 'management' is a hassle. This gives the potential hacker plenty of opportunities to break the password.
